OAR 437-001-0455: Application for Consultative Services

Employers may make a verbal or written request for consultative services. A request must: (1) Be made by an employer to a representative of the Division; (2) Identify the employer and the location where the consultation is desired; and (3) Define the specific problem or hazard, or other reason for the request.
