OAR 409-022-0015: Health Care Facility Annual Reports

(1) The Authority may require an annual report from each licensed health care facility on utilization of the facility. (2) For hospitals, the Authority adopts the American Hospital Association annual survey, as administered by the Oregon Association of Hospitals and Health Systems, as the annual report on utilization of the facility. (3) For all other health care facilities, the Authority shall notify the health care facility of the requirement no later than December 31 of the year to be reported.¶ (4) A health care facility must submit the annual report on a form, prescribed by the Authority, no later than April 30 of the subsequent year. (5) The Authority shall acknowledge acceptance of the annual report or inform the licensed health care facility in writing of any corrections required within 30 days of the submission of an annual report by a health care facility.
