OAR 291-058-0060: Reporting of Sanctions for Parole and Post-Prison Supervision Cases and Role of the Supervisory Authority and Board of Parole and Post-Prison Supervision

(1) Whenever administrative sanctions are imposed in a parole or post-prison supervision case, the supervisory authority or the Board of Parole and Post-Prison Supervision shall be notified utilizing a Department of Corrections violation and structured sanction reporting form. (2) When custody is imposed or conditions of supervision are modified, a completed violation and structured sanction reporting form and Notice of Rights form will be submitted to the local supervisory authority or the Board of Parole and Post-Prison Supervision. (3) Notification shall be sent utilizing the automated structured sanction module within the Corrections Information System whenever possible or via email or facsimile where available during the same working day in which a sanction is imposed. Where email or facsimile is not available, notification shall be mailed the same working day in which the sanction is imposed.
