SLR 11-9.075: MINOR GUARDIANSHIP

(1) Within thirty (30) days after each anniversary of appointment, a guardian of a minor shall file a written report with the court. Copies of the guardian’s report must be given to those people specified in ORS 125.060(3). The report shall be in substantially the same form as that described in ORS 125.325. (An example report is attached as set forth in Appendix 5.)
