ORS 87.575: Lien docket.

Each recording officer shall maintain a lien docket in which, upon the filing of a notice of lien, the recording officer shall enter the name of the injured person, the approximate date of the hospitalization services or medical treatment, the name and address of the hospital filing the notice and the amount claimed and the name and address of the physician, physician associate or nurse practitioner filing the notice and the amount claimed.
