As a public document, the Consent Agreement for Re-entry to Practice may be released to the public. However, the Consent Agreement for Re-entry to Practice is not an adverse action. (3) The Applicant may be granted a license once the Consent Agreement for Re-entry to Practice is signed by the Executive Director or Medical Director. (4) The Executive Director or Medical Director has the authority to terminate a Consent Agreement for Re-entry to Practice upon licensee’s satisfaction of the terms or licensee’s failure to satisfy the terms within the agreed-upon time.
