OAR 833-075-0010: Effective and Expiration Dates of Licenses

(1) Initial licenses will be effective for no more than thirteen months, expiring on the last day of licensee’s birth month. (2) Date of issue will be the date staff reviews and determines that all qualifications for licensure are met. (3) Licenses will not be issued without payment of the required initial license or renewal fees.
