OAR 833-020-0071: Reapplication

(1) Former applicants who reapply for licensure must submit a new, fully completed application form and application fee to the Board. (2) Applicants may request to transfer previously filed documents to the new application file if the documents have not been destroyed under state records retention schedules and are still in the possession of the Board. Such documents include, but are not limited to, documentation of supervised work experience, examination results, and transcripts.
