OAR 411-348-0370: Personal Property

(1) A program provider must prepare and maintain an accurate individual written record of personal property that has significant or monetary value to each child as determined by a documented ISP team or parent or guardian decision. (2) The record must include: (a) The description and identifying number, if any; (b) Date of inclusion in the record; (c) Date and reason for removal from the record; (d) Signature of in-residence caregiver or staff making each entry; and (e) A signed and dated annual review of the record for accuracy.
